ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY
I, on behalf of myself and all minor children in my care (collectively the “Children”) in attendance at the
facilities of IJAM, LLC and/or JAMNOLA (hereinafter collectively, “IJAM”), agree to the following:
ASSUMPTION OF RISK. I understand that the activities that IJAM will provide may involve risks. Those risks
may include slipping, falling, injury from falling objects, fainting, shock, seizure, and allergic reactions. In spite of the risks mentioned above, I freely want to participate and want the Children to participate in the activities provided by IJAM. I understand that these activities may not be supervised and that, in the event of injury, medical services may not be readily available. Neither I nor any of the Children are suffering from any mental, emotional, or physical condition precluding participation in any activities that IJAM will provide. I, on behalf of myself and the Children, assume all of the risks of participating in any activities provided by IJAM and/or on its premises. I agree that none of the Children will be left unattended.
RELEASE AND WAIVER OF LIABILITY. I, on behalf of myself and the Children, hereby WAIVE AND
RELEASE IJAM and its agents, employees, officers, directors and members of and from any and all claims,
demands, causes of action, lawsuits, damages, and liabilities of every kind and nature arising from or in any way related to my or their participation in any of the activities provided by IJAM, including any matters caused by the condition of the physical facility.
LOST, DAMAGED, AND STOLEN ITEMS. IJAM has no responsibility for lost, damaged, or stolen items.
PHOTOGRAPHS AND VIDEOTAPE. I, on behalf of myself the Children, grant IJAM the right to photograph,
videotape, and/or record me and/or any of the Children, and to use my and any of the Children’s name, face, likeness, voice, and appearance in connection with exhibitions, publicity, advertising, and promotional materials
without reservation or limitation, and I, on behalf of myself and the Children, waive any right to royalties or
other compensation arising from any such use.
SEVERABILITY. If any provision or portion of this RELEASE AND WAIVER OF LIABILITY shall be held
to be illegal, invalid, or unenforceable, the remaining provisions or portions shall remain in full force and effect.
I, ON BEHALF OF MYSELF AND THE CHILDREN, ACKNOWLEDGE THAT I HAVE CAREFULLY
READ THIS RELEASE AND WAIVER OF LIABILITY AND THAT I UNDERSTAND THE TERMS SET
FORTH HEREIN AND THAT I UNDERSTAND THAT IT IS A RELEASE OF LIABILITY AND A WAIVER
OF RIGHTS THAT I OR ANY OF THE CHILDREN MAY HAVE.
